Does this label carry what the application filed?
Leave a field empty if the application did not file it. It will not be checked. The health warning is checked either way, so you can leave the whole form empty and just ask about that.
The CSV needs a first row of column names. One column must be image, naming a file inside the ZIP; the others may be beverage, brand, class, producer, address, origin, abv and net_ml.
